Vouchers for $10 February 23^rd
1-3pm

 

Must Bring:

- Proof of income (WIC card, W-2, etc.)
- $10 per voucher (Cash Only Please)
- A valid photo ID that shows your current address (Must be a Forsyth County Resident)
- Proof of rabies vaccine (Certificate of Vaccination) for each pet (dogs or cats) needing a
voucher. 

You will need to pay an additional $5 for a rabies vaccine if your pet(s) is not up to date or if you cannot provide documentation.

Specifics:

- Please DO NOT bring your pet(s).
- Each household is limited to 3 vouchers.
- Only pets less than 7 years of age are eligible.
- NO Credit Cards, Debit Cards, or Checks will be Accepted.
